To request your GED transcript:

• Download a transcript request form in PDF or Word format.
  
  
• Complete the downloaded form. The following information is required:
  • Name at time of testing
  • Current mailing address and telephone number
  • Year of testing
  • Date of Birth
  • Social Security Number
  • Signature and date
  • Information on where to mail transcripts
    
    
• Mail or fax* the completed form to:

Transcripts will be mailed within a week in most cases.

*Please note that we cannot accept e-mailed transcript requests.
